When Kenjaku approached the civilians to escort them out of the Culling Game, some people heeded his words and left with him, while others did not. If you recall Jujutsu Kaisen Season 3, Episode 9 the episode where Kenjaku was leading everyone out of the Culling Game you’ll remember that for those who did leave, their encounter with Kenjaku felt merely like a dream. This fact was even confirmed by Takeshi Iguchi, a member of Yuji Itadori’s Occult Research Club.

 Because the experience felt like a dream, people didn't take Kenjaku seriously and simply went about their lives as if nothing had happened. However, there were also those who refused to listen to Kenjaku they declined his offer to leave the Culling Game because they were unwilling to abandon their homes and property at any cost even if it meant risking their very lives.

This deadlock was formed by Kurourushi a Special Grade Cockroach Cursed Spirit along with Ryu Ishigori, Dhruv Lakdawalla, and Takako Uro. This is known as Sendai Colony Four way deadlock. If this deadlock not occurred, all those civilians would have been killed long ago at the hands of the Cursed Spirit, Kurourushi. Kurourushi had a particular fondness for the iron found in human blood however, despite his desire to kill, he refrained from doing so because he feared Dhruv Lakdawalla. This was because Dhruv Lakdawalla was an incredibly powerful Sorcerer. As long as Dhruv Lakdawalla remained alive, Kurourushi stayed in a Dormant state had Kurourushi made any move, he would have been instantly eliminated by Dhruv Lakdawalla's Shikigami. Furthermore, it wasn't just Kurourushi Dhruv Lakdawalla, Ryu Ishigori, and Takako Uro were equally unable to make a move. If any one Sorcerer shifted their position, they would encroach upon another Sorcerer's territory, triggering a battle among the four a conflict from which no one would emerge alive, nor would anyone gain any advantage. Consequently, all four Sorcerers maintained their respective designated zones. It was precisely for this reason that the civilians survived; they were situated within or near Dhruv Lakdawalla's territory, and no Sorcerer could cross into another's zone. 

Why Yuta Okkotsu Killed Dhruv Lakdawalla Saved the Civilians?

Had Yuta Okkotsu not defeated Dhruv Lakdawalla, every single civilian would have perished regardless. This is because they possessed "points" a resource coveted by every player in the Culling Game yet, being ordinary civilians, they stood no chance of fighting back against Sorcerers. Even if no Sorcerer had actively killed them, they would still have met their end unable to gain points on their own, the rules of the Culling Game would have eventually stripped them of their Cursed Techniques. Thus, one could view those who killed the civilians were the ones saving them.
